Where can I get a model release form in Japanese?

I'm traveling to Japan next year, and intend on taking many photos while I'm there. I'm not skilled enough yet to write my own form or translate one into Japanese.

2. well you may want to try http://photojpn.org/ and join one of their blogs or groups. You could perhaps also try contacting the webmaster Philbert Ono, he could find one for you.
3. 1) Get a good english version of the photo release and if this is used overseas(not in america) make sure all the legaities are covered for that country. 2) Go to a local college and see the language professors there and see if one of the students there might want to help. There are probably Japanese nationals there as well, so they may be willing to help.
